About Us

Who we are and why Heartfield exists.

Our mission

Heartfield Community Living is being built around a simple belief: every person with an intellectual or developmental disability deserves a place to call home—a place where they are treated with dignity, have a voice in their daily lives, and feel connected to their community.

Our story

Every person experiences home differently. That understanding began to take shape during my work in healthcare. Patients came in for many reasons, and as part of the safety and security department, I sometimes spoke with them about their living situations and the challenges they faced day to day.

What people shared underscored how personal support is. Routines, preferences, strengths, and challenges vary from person to person. A diagnosis or support need can offer important context, but it cannot define someone's idea of a good life.

I wanted to help build an approach that starts with each person and shapes support around their circumstances, priorities, and vision for a meaningful life. That belief is the foundation of Heartfield Community Living.

I'm a U.S. Army veteran. After my service, I pursued a career in sales and logistics, including commercial transportation and freight brokerage. It provided stability and valuable experience, but over time I realized I was searching for something more meaningful.

I had long been interested in becoming a hospital chaplain, and as I followed that path, I found myself increasingly drawn to healthcare. That journey led me to a position in Safety and Security at Norton Healthcare, where I gained firsthand insight into the importance of dignity, compassion, and service.

Executive Director

We are looking for an experienced Executive Director to lead day-to-day operations at Heartfield Community Living. This page will introduce them once the position is filled.

Who We Serve

Heartfield is being built for people who deserve a real home, and for the families and professionals who care about them.

Heartfield is not open yet and is not accepting residents at this time. Our Kentucky SCL provider certification is in progress.

The people we will support

Individuals with intellectual and developmental disabilities who want to live in a home setting with support around the clock. Every person's support is built around their own goals, routines, and preferences.

Families and guardians

We see families and guardians as partners. You will always know who is caring for your loved one, and you are welcome to visit, ask questions, and help shape their plan.

Case managers and support brokers

We will work closely with case managers to coordinate services, share updates, and make sure each person's plan is followed and reviewed.

Eligibility

Services will follow Kentucky Medicaid waiver requirements. If you aren't sure whether Heartfield is a good fit, reach out and we'll talk it through.

Daily Activities

A full life at home and in the community, shaped by each person's choices.

A typical day

  1. MorningWake up at your own pace, get ready with help as needed, and share breakfast together.
  2. DaytimeWork, day programs, or time out in the community, plus practice with everyday skills at home.
  3. EveningDinner together, then free time for hobbies, games, TV, calls, or visits with family and friends.
  4. OvernightAwake staff on site through the night, so help is always close by.

What residents can look forward to

Life skills

Cooking, laundry, cleaning, shopping, and managing money, learned step by step and at each person's pace.

Community outings

Parks, the library, local events, restaurants, and errands around town.

Recreation and hobbies

Music, art, games, movies, sports, and gardening, based on what each person enjoys.

Health and wellness

Walks, exercise, healthy meals, and support getting to medical appointments.

Faith and culture

Support attending a place of worship or cultural events, if the person chooses.

Family and friends

Visits, phone and video calls, and help staying connected with the people who matter most.

Is Heartfield open now?

Not yet. Our Kentucky SCL provider certification is in progress, and we are not accepting residents at this time. We are happy to answer questions and talk with families now.

How are services paid for?

We plan to provide services funded through Kentucky Medicaid waiver programs, such as SCL. A person's case manager or support broker can help confirm their eligibility.

Is staff there around the clock?

Yes. We plan to provide 24/7 support, including awake staff overnight.

Can families visit?

Yes. Families and guardians are welcome, and we will work with each resident on visits that fit their schedule and wishes.

How is each person's care planned?

Every resident has a person-centered plan built with them, their family or guardian, and their case manager. It covers their goals, routines, and support needs, and it is reviewed regularly.

A day at Heartfield

Here is what daily life will look like once our home opens. Every resident's day is shaped by their own plan, interests, and choices.

  1. MorningResidents will wake up at their own pace, with staff helping with personal care as needed, then share breakfast together.
  2. DaytimeWork, day programs, or time in the community: shopping, appointments, the library, parks, and local events. At home, residents practice skills like cooking, laundry, and managing money.
  3. EveningDinner together, followed by free time for hobbies, games, TV, phone calls, or visits with family and friends.

Direct Support Professional

Full-time and part-time positions, including day, evening, overnight, and weekend shifts.

What you'll do

Help residents with daily routines, cooking, and personal care. Support them in reaching their goals, join outings in the community, and keep simple daily records.

What we're looking for

At least 18 years old with a high school diploma or GED. Able to pass a background check. A valid driver's license is a plus. Most of all: patience, kindness, and reliability.
